Initial Exam

During a thorough examination of the gum tissues, the doctor will collect data related to bone levels, pocket depths, bleeding points, and overall gum health. This examination will enable your doctor to provide you with the appropriate diagnosis, prognosis, and treatment plan for your specific needs. We will communicate these findings to your general dentist as well. Occasionally, surgery can be done the same day as the examination. However, a complex medical history and treatment plan may require a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any x-ray images if applicable
  • All the completed forms provided to you prior to your initial exam
  • If you have dental insurance, bring the necessary documentation. This will save time and allow us to help you process any claims.
  • IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the examination visit.

    Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. heart murmur, history of rheumatic fever, diabetes, high blood pressure or any artificial heart valve or joint) or if you are on any of the following medication (i.e. heart medications, coumadin, steroids, aspirin, anticoagulant therapy, etc.)

Financial/Insurance

At the time of your initial visit, your treatment plan will be established and an estimate of fees will be given to you.

Our office is committed to providing excellent dental care and guide patients in choosing the best payment option for their individual needs. We accept cash, check, Visa, Mastercard, American Express, Discover and Debit cards. In addition we offer extended payment policies, often without interest charges through the dental service financing company CareCredit.

Dental Insurance

Your insurance is a contract between your employer and a dental insurance company. Benefits received are based on the terms of the contract negotiated between your employer and the dental insurance company, and not your dental office.

The goal of most dental insurance policies is to provide only basic care for specific dental services. The benefits that you receive from the dental insurance company have nothing to do with your achieving high quality, comprehensive treatment.

Your benefits are decided between your employer and the insurance company. Many needed services may not be covered. Our office will do everything possible to help you understand and make the most of your dental insurance benefits. As a courtesy, our office will file your insurance claims for you. Upon receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you.

If you have any problems or questions, please ask our staff. They are well informed and up-to-date. Please call if you have any questions or concerns regarding your initial visit.

As a courtesy to our patients, we will assist you in obtaining benefits for all services delivered. A formal request to our office is required if you desire a Pre-Authorization of your insurance benefits. Your dental insurance carrier may pay less than the actual fee for services. Therefore, all fees incurred are ultimately your responsibility regardless of insurance benefits.
